For Patients
Walnut Creek Medical Group is happy to help you with any questions you may have prior to your visit or prior to making an
appointment with us. We are committed to providing the most up-to-date healthcare services to our patients.
Appointments
We see all patients on an appointment basis only, and ask that you call or e-mail in advance, using the Relay
Health (www.relayhealth.com) service, so that we may reserve an appointment time for you. We try to see all patients in a timely manner. If you cannot keep an appointment, please call and notify us
as soon as possible. We greatly appreciate 24-hours notice so that we may better serve our other patients.
Your First Visit
At the time of your visit, our office staff will update our system records to ensure we have current and accurate contact
and insurance information. Your physician will review your past medical history and address any current issues you may have. For your convenience and to make your visit more effective, please note the following:
- If you have received Patient Information and Medical History forms in the mail, or downloaded them in advance of your
visit, please complete them and remember to bring them with you. Download forms:
- Bring a list of questions and concerns.
- Bring a list of all vitamins and medicines you are using, including the dose and frequency of the medication(s).
- Please bring your insurance card and be prepared to pay any co-payment required by your insurance plan. (Note: Your appointment
will may be rescheduled if you cannot provide your insurance card or information.)
- Bring a photo ID.
Cancellation/No-Show Policy
For appointments not kept or cancelled at least 24 hours in advance, there is a variable fee, up to $100.00. This is not a charge paid for by insurance companies. After three
missed appointments, you may be released from the practice.
Medical Insurance
We accept most insurance plans. If you have an insurance that our office is not contracted with, please inquire about financial
arrangements so we can meet your health care needs.
Medical Emergency and Urgent Care
In the event of a life threatening emergency, call 9-1-1 or go immediately to the emergency room of the nearest hospital
and ask them to contact your doctor. For non-emergency situations, please call our office during the hours of 9-12 and 2-5.
If our office is closed, you may speak with the answering service and ask to speak with the on-call physician. Please identify
who your doctor is and briefly describe what the problem is. The answering service will contact the doctor on call and your
call should be returned within 1 hour.
Telephone calls
We encourage you to call with questions you may have concerning your health care needs. Your physician will do her best
to answer your call within 24 hours, but usually by the end of the business day. You may also e-mail your physician via www.relayhealth.com.
Prescription Refills
If you need a refill on an existing prescription, please call your pharmacy at least 3 days before you need the medication.
The pharmacist will fax us the request if there are no refills left. Your doctor will review your chart and either approve
the refill or call you to set up an office visit. Prescription refills should take 3 business days or less to fill. If you have
not received your medication refill approval within 72 hours, please call our office.
Your Medical Records
Medical records are electronically and securely stored and are strictly confidential. Records will be released only upon
your written authorization. Records will be mailed or available for pickup within ten business days of receipt of your request,
unless an urgency exists which requires they be sent sooner. There is a fee to copy records, unless being sent directly
to another physician's office.
